Faculty Appointments
If you currently serve or have served as a full-time, part-time or adjunct faculty member of a Medical or Optometry school within the past 10 years,
list the schools below with the beginning and ending dates of your appointments.
To display a list of medical or optometry schools in the New Jersey area , click on the arrow.
Locate the name of the school and click on the name.
If the school is not found on the list, type the name of the school in the block below.
Enter date appointment started. (Example 03-25-2015)
Enter date appointment ended. (Example: 03-25-2015)
To add an additional medical school,
Click on 'Click here to add additional Medical or Optometry school appointments' button, and proceed as above.
(You may add multiple medical school appointments.)
When finished, click one of the buttons at the bottom of the screen:
Save & go to previous page
Save & stop for now
Save & go to next page
