Getting Started
To begin the Profile, enter your Medical or Optometrist License Number and your password.
If this is the first time that you have accessed this web site or have not yet successfully passed the authorization process,
the password you will enter is the temporary password contained in the initial mailing packet that you received. See authorization page.
Enter your 12 character Medical or Optometrist License Number.
The license number is not case sensitive.
(enter only the letters and numbers, leaving out any spaces i.e. 25MA12345600 or 25ma12345600).
Enter the temporary password you were supplied, or if you have already completed the authorization process, enter the new password you created upon successful authorization.
The password is case sensitive.
Click on the 'Login' button to continue.
If this is your first access of the Practitioner Health Care Profile web site or you have not successfully completed the authorization process,
you will automatically be directed to an authorization page that will request additional information for confirmation.
