Authorization
Additional information is required in order to activate your account.
Enter your date of birth. (Example: 03-24-1970)
Enter the last 4 digits of your Social Security Number.
Click on the 'Submit' button.
If the information you have entered matches the information that is on file,
you will be directed to change your temporary password.
Choose a password that is six to eight characters long.
It must contain only alphabetic or numeric characters (a - z, 0 - 9).
Your password must begin with an alphabetic character and must contain at least one number and four alphabetic characters.
